2005 Reports for Agencies

After each region is reviewed, the Community Monitor assembles the resulting data into a Power Point presentation, which is shared with the Regional Office staff and provider agencies of the individuals reviewed. These presentations are now available through the links below, in DRAFT form as presented during the regional provider meetings. These DRAFT presentations can be viewed online, downloaded and printed.

Please note: Each presentation is quite large, and will take time to download,

depending on your internet connection speed. Thank you for your patience with this process. Also, although the original format for each presentation was PowerPoint, you do not have to have PowerPoint for viewing, as each will open in your browser window.

The scores from the completed protocol books are evaluated by the Community Monitor and become the basis for the Regional and Statewide reports.

The protocol scores are also broken down by region and are then evaluated in three areas: Case Management Agencies, Day Services Providers and Residential Services Providers. The scoring sheets also provide a comparison of the agency scores from 2004 to 2005.
